You may scan and send a completed application to cityclerk@mendoncityutah.gov, bring in (or fill out in person) to the clerk’s office during regular office hours, drop off in a sealed envelope marked Attn. City Clerk in the water drop box located on the south side of the parking lot at city hall or mail to Mendon City Clerk PO Box 70 Mendon, UT 84325.
